Confidentiality & Medical Records
We are a research practice and we ask you for information about yourself so that you can receive proper care and treatment. We may use some of this information for other reasons, for example - to help us protect the health of the general public, to plan for the future, to train staff and to carry out medical research for the benefit of everyone. All information used is done so anonymously.
On occasions we are involved in research studies which are, again, anonymous. If anything to do with this research involves you personally we will contact you to see if you are willing to take part.
Everyone working within the NHS has a legal duty to keep information about you confidential.
All doctors and nurses record details of consultations on your electronic medical records. This information is only available to all those persons involved in your healthcare. You can arrange to see you records by contacting the practice manager - a charge may be made for this facility.
Information about the General Practioners and the practice required for disclosure under this act can be made available to the public. All requests for such information should be made to the practice manager.
We need to hold personal information about you on our Computer system and in paper records to help us to look after your health needs, and your doctor is responsible for their accuracy and safe-keeping. Please help to keep your record up to date by informing us of any changes to your circumstances.
Doctors and staff in the practice have access to your medical records to enable them to do their jobs. From time to time information may be shared with others involved in your care if it is necessary. Anyone with access to your record is properly trained in confidentiality issues and is governed by both a legal and contractual duty to keep your details private.
All information about you is held securely and appropriate safeguards are in place to prevent accidental loss.
In some circumstances we may be required by law to release your details to statutory or other official bodies, for example if a court order is presented, or in the case of public health issues. In other circumstances you may be required to give written consent before information is released – such as for medical reports for insurance, solicitors etc.
To ensure your privacy, we will not disclose information over the telephone or fax unless we are sure that we are talking to you. Information will not be disclosed to family, friends, or spouses unless we have prior written consent, and we do not leave messages with others.
Access to Records
In accordance with the Data Protection Act 1998 and Access to Health Records Act, patients may request to see their medical records. Please ask at reception if you would like further details. An appointment will be required. In some circumstances a fee may be payable.
We all do our best to give a courteous and sensitive service to patients but on occasions things do go wrong. If you have any cause for complaint please contact our practice manager, Ms Nadina Prestedge, using any of the contact methods above.
PALS (Patient Advice and Liaison Service) is available to offer confidential advice, support and information on health-related matters. Leaflets are available in the waiting room giving further information on the service.

Similarly please contact us if you have any suggestions on how we might improve our services. We also have a suggestion box in our waiting room for your comments (which can be anonymous if you prefer).
As part of our ongoing drive to improve services we often ask patients to complete information forms or questionnaires. We greatly appreciate your help with these.
We also also produce a monthly newsletter which appears in the community and in local parish magazines in the surrounding villages.
This surgery operates a zero tolerance policy towards verbal and physical abuse towards any members of our staff. Patients are asked to be considerate and act reasonably. all incidents will be followed up and you will be be sent a formal warning or removed from the practice list if your behaviour has been unreasonable. We will have no hesitation in having you removed from the building by the police should your behaviour warrant it.

The surgery is committed to both eliminating discrimination and encouraging diversity amongs our workforce and in relation to all our visitors and aims to ensure all visitors and staff are treated with dignity and repect.
The surgery will not tolerate any form of discrimination or harassment of our staff by any visitor. Any visitor who expresses any form of discrimination against, or harassment of, any member of our staff,, will be required to leave the Practice's premises forthwith. If the visitor is a patient, he/she may be removed from the Practice's list if any such behaviour occurs on more than one occasion.
Equality & Diversity Policy
In May 2010 we were pleased to pass our assessment for the Practice to become a training practice, with Dr Barber as qualified GP Registrar trainer. GP Registrars are fully qualified doctors who hope to pursue a career in General Practice and as part of this need to gain experience in a surgery.
